Our Aim Is to review all of our patients annually who are on a repeat medication, especially those with long term conditions or who are on multiple medications.

Our Aims for Medication Reviews

We carry out annual medication reviews to ensure the best care for our patients. Our aims are to:

Keep you safe by checking that your medicines are still suitable for your health needs.
Improve effectiveness by making sure your treatment is working as intended.
Reduce risks by identifying and preventing possible side effects or interactions.
Support understanding so you know how and when to take your medication correctly.
Update your care plan if your health or circumstances have changed.
Optimise medicine use ensure appropriate prescribing & reduce adverse events.

Medication Reviews

All our patients on regular medication will need an annual medication review. This is an Important check to make sure:

  • Your medicines are still safe and effective
  • The dose and type of medication are appropriate for your health needs.
  • You understand how to take your medication correctly.Your health can change over time, and some medicines may need adjusting or stopping. Reviews help prevent side effects and keep your treatment up to date.

Reduction Policy

If you do not attend your annual medication review, we may need to reduce the quantity of medication we prescribe at one time. This is because:

  • Safety first – We need to check your medicines are still suitable for your health.
  • Prevent side effects – Regular reviews help us spot any problems early.
  • Ensure correct doses – Your health can change, and your medication may need adjusting.
  • Legal and clinical responsibility – Prescribing without review could put you at risk.

Reducing the amount ensures you have enough medication while encouraging timely reviews to keep your treatment safe and effective. If you have concerns, please contact our reception team.

We will send you a text or email when you are due a medication review with a form attached to fill out, Or you can click on this link (Patient Appointments – WHITBY HEALTH PARTNERSHIP) and this will be reviewed by our clinical pharmacists.
