Home Visit Policy

– Due to increasing demands, Clinicians can no longer automatically visit any patient who requests a home visit.
– Requests that come in after 10:30am will not be accepted and may be triaged the following day.
– All visits must now undergo an element of triage and be dealt with according to clinical need.
– A Clinician will only visit you at home if they think your medical condition requires it.
– A Clinician will also decide how urgently any visit is needed. Clinicians are better able to assess patients at the surgery where they have access to your full medical record including allergies, medications, results etc. as well as specialist equipment, good lighting and examination facilities and therefore the surgery is always the preferable site for any consultation.
– Clinicians who are having to visit inappropriate house calls are delayed from visiting those patients who are in genuine need of a house call.
– Clinicians are not obliged to visit a patient if they have assessed the patients’ clinical need on the telephone and found them to be suitable for an alternative method of healthcare. As long as a GP or appropriate healthcare professional has provided a plan for a patient (which may be an appointment the same day, a future day, telephone advice or attendance at another healthcare site) the partners of the surgery will support the decision made.
– Home visits are reserved for the following: patients who are genuinely housebound and who do not leave the house for any reason; including those in nursing and residential homes; Patients who’s clinical condition prevents them from travelling to the surgery and that they have a medical condition that necessitates an urgent medical opinion;
– Age is not a criterion for a home visit.
– Home visits will not be undertaken for social / transport reasons.
– The GP / Clinician may refuse a visit to the patient and offer an urgent appointment at the surgery if they feel that is more appropriate.
– Patients should dial 999 in the case of a genuine life-threatening emergency. For life threatening emergencies, requesting a home visit from a GP can delay life-saving treatment.
– It is always appreciated if you can highlight any obvious risks to clinicians when requesting your home visit i.e. Animals in the home, Smoker etc. This allows us to risk assess in advance. Expectations would be that the consultation area is smoke free, there is a space to safely enable the clinician to undertake their role, or that any animal that is likely to cause a nuisance is not in the same room for the duration of the visit.

Click “Submit a request” above to launch our online consultation platform. You can use this to request a NON-URGENT request or appointment at the surgery.

Avoid waiting on the phone with this service. Your request will be triaged and you will receive a response within 24 hours (usually much sooner).

Whitby Health Partnership provide 3 types of appointments:

Acute – Urgent / on the day

Routine – Not urgent but important

Specialist – Chronic disease management and specialist clinics e.g. Family planning or immunisations

-Our patient services team will help to triage your query to the most appropriate service

-Sometimes you may be signposted to another local service provider that can see you sooner e.g. eye clinic or Pharmacy

-Some requests can be dealt with remotely and do not require a face to face consultation.

-Urgent cases will be dealt with on the same day.

  • For in-person consultations, we provide you with a 15 minute appointment to discuss your most important concern. Unfortunately, we cannot discuss multiple concerns.
  • We are happy for you to request a specific clinician, however, it may result in a slightly longer wait before your appointment.
  • If you are unable attend an appointment or wish to cancel. Please cancel using the NHS AppPatient Access App or via the quick cancel button.
  • The surgery does not offer a walk-in service and is strictly by appointment only.
  • If you arrive late for an appointment, it may not always be possible to fit you in, so please allow adequate time for parking and registration prior to your appointment.

Non-urgent advice: Enhanced Access Appointments

Whitby Health Partnership is a part of One Ellesmere Port Primary Care Network (PCN). The PCN is a collaboration of the 6 GP Practices from across the town who are working together to develop better services for our patient population.

Our PCN now provides additional appointments during weekday evenings and on a Saturday:

Monday – Thursday: 6:30pm to 8:00pm
Friday 6:30pm to 8:00pm (Held in Great Sutton Medical Centre or York Road Group Practice)
Saturday: 9:00am to 5:00pm (Held in Great Sutton Medical Centre or York Road Group Practice)

These appointments are available for patients registered at all 6 GP Practices and therefore you maybe asked to attend an alternative GP Practice, if suitable.
You can book these appointments in the usual manner via Whitby Health Partnership.

Late For Your Appointment

Please attend your appointment on time and allow sufficient time for parking as the carpark can get busy at peak times.  If you are late we may not be able to see you.

Text Reminder Service

We have a texting service which allows you to receive confirmation and reminders about your appointments.

To have this service you will need to register by completing a consent form.

Please remember to update your contact details with us when you change address, telephone numbers and email address.
