All new patients are requested to complete a number of documents as part of the registration process which can be found by clicking New Patient Registration Forms

Paper copies are available at the reception but during the current COVID-19 (coronavirus) crisis we are requesting that visiting the surgery should only be if completely necessary.

Please remember it is up to the doctor to decide if they will accept you as a patient.

Named Accountable GP

Every patient in the practice has been allocated a named GP who is responsible for your overall care. The named accountable GP will take lead responsibility for the coordination of all services required under the contract and ensure they are delivered to each of their patients where required.

If you do express a particular preference as to your choice of accountable GP we will try to accommodate this request.

Please remember that this does not stop you seeing a GP of your choice, as you currently do.
