Adults who suspect that they or someone they care for may have Attention Hyperactivity Disorder (ADHD) or Autism (ASD) can undergo a formal assessment. Adults can self refer via the NHS England Right To Choose pathway.

Non-urgent advice: For Self – Referral
To self-refer for an assessment, follow steps 1 and 2 below, and send us your completed paperwork to our secure email.
This will be reviewed by your GP and you should have a response from the surgery within 14 days informing you if your referral has been forwarded to your chosen provider.
Before you complete steps 1 and 2, you must first choose a provider in order to complete the forms with your providers details.
Please note: It can take up to 12 months to obtain an assessment date. Only contact the surgery after the estimated wait time has been reached. This can be found on your chosen providers website.

2. Download and amend this letter to include your details and address it to your GP

Non-urgent advice: Shared Care Agreements for ADHD and ASD
What is a Shared Care Plan?
A Shared Care Agreement (SCA) is a formal, written agreement between you (the patient), your GP, and your specialist (such as a psychiatrist) to jointly manage your care for Attention Deficit Hyperactivity Disorder (ADHD) or Autism Spectrum Disorder (ASD). It typically applies once your medication has been stabilised by the specialist, allowing your GP to take over routine prescribing and monitoring while the specialist provides ongoing oversight and advice. This arrangement follows NICE guidelines and helps ensure safe, coordinated care.

GP Responsibilities:
• Prescribe your stabilised medication dose and arrange repeat prescriptions.
• Monitor your health, including blood pressure, pulse, and BMI.
• Review any test results or concerns and liaise with your specialist as required.
• Ensure you understand your treatment and report any issues promptly.
Patient Responsibilities:
• Attend all appointments with your GP and specialist, including annual reviews.
• Take your medication as prescribed and report any side effects or changes in health immediately.
• Inform your GP and specialist of any other medications (including over-the-counter) or lifestyle changes.
• Store controlled medications securely and comply with legal requirements (e.g., for driving and DVLA notifications if applicable).
Right a right to choose or Private Assessment
Your GP will only continue prescribing medication under a Shared Care Agreement for as long as your specialist maintains oversight of your medication and conducts annual reviews (or as clinically required). If these are not continued, or if there are clinical concerns, prescribing may be paused, and care will revert to the specialist. All shared care is voluntary and subject to our practice’s capacity and clinical judgement. Shard Care Agreements will be treated the same for providers on the Right to Choose list, as well as Private Providers.
