One Ellesmere Port

Who we are?

The One Ellesmere Port Primary Care Network was established in 2018. Its members include 6 GP Practices from across the town who are working together with the common goal of creating a better future for their patient population.

The practices that form our Network are:
Great Sutton Medical Centre
Old Hall Surgery
Whitby Health Partnership
York Road Group Practice
Hope Farm Medical Centre
Westminster Surgery

Together we are working together to create a better future for over 68,000 patients across Ellesmere Port. Easier navigation around the health and social care system, is our first important strategic aim.

In 2018 NHS England published the Long Term Plan which identified 7 aims for the future of our health service. As we develop as a Primary Care Network we are committed to delivering these aims for the population of Ellesmere Port. The aims are:

Medication reviews and optimisation
Anticipatory care
More personalised care
A better offer for patients in care homes and residential care
Earlier detection and diagnoses of cancer
Action on prevention of cardiovascular disease
Action to tackle health inequality.

Most importantly we recognise that the above cannot be delivered by General Practice alone. Since April 2019 we have been working collaboratively with local organisations such as Local Authority, Health Watch, Trinity Church and Cheshire and Wirral Partnership Foundation Trust to identify ways in which we can work together to improve services.
Our journey has only just begin, but we are passionate about Ellesmere Port and together we will make a difference.
